Please read this contract carefully as it is a legally binding document. Initial all paragraphs and complete the last page. All fields are mandatory. If you cannot fill out a section or disagree with the contents of any portion, please contact It Takes A Village Rescue to discuss. Please be sure you understand decompression and the time it takes to allow your new family member to adjust to your home. Adoption fees are non refundable. Please obtain a martingale collar AND an airtag or simliar tracking device for your dog. A martingale and airtag/similiar device is required to be used when bringing your dog home to prevent accidental loss of the pet. If your animal is lost on the trip home or after arriving home immediately call us. Do NOT pursue chase, you will push the dog further away. Drop down to the ground and observe direction the dog goes. As soon as the dog feels it is away from people it will most likely stay put. The worse thing you can do in chase or yell to the dog. Terms & Conditions*
- I am adopting the pet described below through It Takes A Village Rescue. The pet is being adopted as a companion and will live as a family member with me inside my home. I will not use the dog for any other purpose or at any time gift, rehome, sell, or surrender to a rescue or shelter.
- I will provide proper and sufficient, food, water, shelter and kind treatment for the pet at all times. I further agree the dog will not be tied out or chained at any time as a means of restraint or be a primarily outside dog.
- I agree that a representative of It Takes A Village may make periodic inspections of the pet, my residence and the pet’s living conditions on reasonable notice. I further agree to surrender the pet to It Takes A Village Rescue's representative if for any reason he/she considers the conditions provided by me for the pet to be unsatisfactory.
- I will keep a collar and identification tag on the dog at all times and will license the dog with the county/city. I agree to immediately register my pets microchip and should my contact information change to imemdiately update the registry.
- I agree to immediately notify It Takes A Village Rescue if the pet is lost or stolen. I further agree, in the event the animal is lost, that I will make every reasonable effort to attempt to locate and claim the animal, and to immediately retrieve the pet from any public pound or animal shelter when notified the animal is being held at any such location and to pay any costs associated with its recovery.
- It Takes A Village has provided me (or will provide me upon arrival of the dog) with the following: Adoption Contract + Health Records. I agree to keep my pet current on vaccines at all times and on year round heartworm prevention. I will have my pet established at my vet within 2 weeks of adoption.
- If behavior issues arise I agree to use a positive reinforcement trainer. I agree to notify the rescue immediately to make them aware of the behavior issue. I agree to NEVER use any sort of shock collar (whether training or fencing) on the dog.
- I understand that in the event I need to surrender the dog I will only do so back to It Takes A Village Rescue, unless It Takes A Village Rescue agrees otherwise. I understand adoption fees are not refundable. I further understand that at the time of returning the pet to It Takes A Village Rescue that a foster home may not be available, at which point I will cover the cost of boarding by the rescue at their vet until they secure an approved foster. I further understand that adoption is a lifelong commitment, and the rescue cannot always be expected to take every animal back. If the rescue cannot secure a foster in 2 weeks than adopter will be responsible for finding and screening an appropriate home on their own or choosing behavior al euthanasia if vet recommended
- If I am adopting a dog too young to be spayed/neutered, I understand that I must spay/neuter by 6 months of age. I agree I will not let me dog go into heat/to the age of reproduction. I further agree that EVEN IF MY VET WANTS TO WAIT UNTIL THE DOG IS OLDER I WILL USE ANOTHER VET WHO AGREES TO SPAY/NEUTER AT 6 MONTHS OF AGE. I UNDERSTAND IF I DO NOT COMPLETE THE SPYA/NEUTER ON TIME I VOID THIS CONTRACT AND HAVE TO RETURN THE DOG TO RESCUE IMMEDIATELY. I agree that I will not delay the spay/neuter for any reason and that even though some vets recommend waiting 1 year that is not an option. Failure to bring the dog to the scheduled appointment, immediately voids this contract and the dog becomes property of the rescue and I must immediately return the dog to the rescue's care. I will pay a $1000 penalty if the dog is not spayed/neutered by 6 months. I understand the rescue will file breach of contract charges and I will reimburse attorney fees to the rescue as well as any other associated fees to include but not limited to, filing costs, time, mileage, postage. I UNDERSTAND I SHOULD NOT SIGN THIS AGREEMENT IF I AM NOT GOING TO SPAY/NEUTER BY 6 MONTHS OF AGE AS IT IS NON NEGOTIABLE
- I understand that the dog I am adopting needs time to decompress and adjust to their new home. I understand that the 1st few weeks the dog should go minimal places, not meet any pet outside the immediate home, and should have minimal meets with people outside the immediate family. The dog needs the first few weeks to adjust and bond to the new family.
